Last case doesn't eject (sometimes)

I'm pretty sure this has been posted before. I have tried, with my limited skills, to search for it but have been unsuccessful. Please bear with me and help if you can.

I am shooting a government model SA 1911. Occasionally, the last casing (always the last casing without exception) does not eject. Before it can completely clear, the slide comes forward and jams it in. Any ideas or experiences will be appreciated.
